How can I get a new Health Card?

You will need a new health card if yours has expired, been lost or damaged, if your name has changed or if you have received notification from the Ontario Ministry of Health that it must be replaced.

If you have lost your green photo card, telephone 1-800-268-1154 (toll-free in Ontario); give the requested information and a replacement card will be mailed to you.

If your green photo card has expired, or if you have lost the red and white card, you must go in person to one of the offices of the Ministry of Health:

Peterborough Robinson Place, 300 Water Street

The offices are open from 8:30 a.m. to 5:00 p.m. Monday through Friday.

You will need to take original documents with you, one from each of the following categories:

Proof of Citizenship or OHIP-eligible status
Proof of
Residency
Proof of
Identity
A document to prove you are eligible for Ontario health coverage. Examples:
  • Provincial Birth Certificate
  • Canadian Registration of Birth
  • Certificate of Canadian Citizenship
  • Permanent Residence Card
  • Certificate of Indian Status
A document displaying your name and current home address. Examples:
  • Ontario Driver's License
  • Mortgage or rental agreement
  • Utility or property tax bill
  • Bank account statement
  • Old Age Security or Canada Pension Plan statement
A document that displays both your name and your signature. Examples:
  • Passport
  • Ontario Motor Vehicle Permit
  • Credit card
  • Employee ID card
  • Student ID card
  • Union card
  • Old Age Security card
If your name has changed you will also need to provide a marriage certificate or change of name certificate. If your citizenship or immigration status has changed, you will need to provide an original citizenship or immigration document.
